Information Provided to the Physician

The employer should provide the following information to the examining physician:

• A copy of section 5218 and its appendices
• A description of the affected employee's duties as they possibly relate to his or her exposure
• The employee's actual or representative benzene exposure level
• A description of any personal protective equipment used or to be used
• Information from the employee's previous employment-related medical examinations
