Information for the Physician or Health Care Provider

The employer must give the following information to the physician or health care provider before he or she can determine whether it is safe for an employee to use a respirator:

  • The type and weight of the respirator
  • The duration and frequency of respirator use
  • The expected physical work effort
  • Any additional protective clothing or equipment to be worn
  • Temperature and humidity extremes that the employee may encounter
  • A copy of the employer's written respiratory protection program
  • A copy of 8 CCR 5144
