Post-Emergency Incident Report

The owner or operator of the facility is responsible for ensuring that all emergency incidents that activate the contingency plan have been noted in the operating record and reported to the EPA regional administrator within 15 days of the incident. The owner or operator must note the time, date, and specific details of any incident that requires activation of the contingency plan.
