Post-Emergency Incident Report
The owner or operator of the facility is responsible for ensuring that all emergency incidents that activate the contingency plan have been noted in the operating record and reported to the EPA regional administrator. The owner or operator must note the time, date, and details of any incident that requires activation of the contingency plan. Within 15 days of the incident, he or she must report the following to the EPA regional administrator:
- Name, address, and phone number of the facility and its owner or operator
- Date, time, and type of the incident
- Name and quantity of the materials involved
- The extent of injuries
- An assessment of actual or potential hazards to human health or the environment
- Estimated quantity and status of recovered materials involved in the incident
