Shipper's Declaration For Dangerous Goods

Complete the shipper's certification on the Shipper's Declaration For Dangerous Goods form, which is a standard commercial form. Two different types of the form are available, using different versions of the "Nature and Quantity of Dangerous Goods" section: one that is open and more free form, while the other uses a column format with headings to handle each key entry.

It is the shipper's responsibility to obtain its own copies of the Shipper's Declaration For Dangerous Goods form, either purchasing it or creating its own as long as the form meets the format, size, and color specifications described in the IATA Dangerous Goods Regulation Section 8 Documentation.

You must complete and sign at least three copies of the Shipper's Declaration For Dangerous Goods form:

  • Attach one certification form to the copy of the manifest placed on the aircraft.
  • Attach one certification form to the originating station file manifest; in transit or en route terminals can create copies of the form for their station file if required.
  • Place one certification form in a waterproof envelope attached to the number one piece of the shipment.

The three original forms must have the specified vertical red hatch border and certifying official's signature (facsimile signatures can be used if applicable laws recognize them as legal signatures); typewritten signatures are NOT allowed. Additional copies can be forwarded with the shipment, but the vertical red hatch border is not required.

You can either complete the Shipper's Declaration For Dangerous Goods form by hand (manually) or by computer or typewriter (digitally or mechanically).

You can also complete it using any combination of ways, as long as all entries are durable, clear and legible on all required copies. However, you should only use one style if possible. You can use either upper or lower case, or a combination.

Shipments may be rejected or encounter issues if any entry is not clear and legible or information in required fields (or keys) is missing.
