Shipper's Declaration Form - Key 1 & 2

Figure A17.1 provides instructions on completing the Shipper's Declaration for non-radioactive and radioactive shipments. Use Figure A17.2 to determine if a Shipper's Declaration form is required for radioactive shipments.

Following are the fields (or keys as described in AFMAN 24-604) to be completed on the form:

• Shipper (Key 1): Enter the full name and address of where the hazardous material was certified.
• Consignee (Key 2): Enter the address and the six digit Department of Defense Activity Address Code (DODACC) and or in-the-clear geographical location of the ultimate consignee (if known), or "Worldwide Mobility" according to A17.3. For infectious substances, include a name and telephone number of a responsible person to contact in case of an emergency.
