Shipper's Declaration Form - Key 19

The following are the fields to be completed on the form:

  • Additional Handling Information (Key 19): If an item poses multiple hazards, enter the PSN, hazard class, and net quantity of each accessorial hazard if specifically required (for example fuel, dry ice). Adding "Class" or "Class or Division" with the hazard classification is options (example: Class 3 instead of 3).
  • Handling instructions, when specified by a packaging paragraph; you only need to enter these if the handling instruction applies to the material being shipped.
  • The control and emergency temperatures for temperature controlled Class 4.1 and 5.2 materials
  • For grandfathered munitions certified according to Attachment 27, add the statement "Government-owned goods packaged before January 1, 1990".

Additional Handling Information (Key 19):

  • If a shipment is packaged and transported under the authority of a CAA (Packaging or Hazard Classification), add "PACKAGING AUTHORIZED BY COMPETENT AUTHORITY OF THE UNITED STATES OF AMERICA (USA)". If a different country issued the CAA, use that name instead of U.S.A. in this key.
  • Enter the text "EMERGENCY CONTACT" followed by the 24-hour Emergency Response number for the hazardous materials listed on the Shipper's Declaration form. (A17.2.9.3 lists Emergency Response numbers for DOD activities.)
