Shipper's Declaration Form - Key 5 & 6

The following are the fields to be completed on the form:

• Shipper's Reference Number (Key 5): Enter the 17 character Transportation Control Number (TCN).
• Optional block (Key 6): Inspection activity will annotate the date of inspection and acceptance for air movement according to A28.1.2. The Shipper unit cargo identification information may also be entered.
• Shipment within Passenger Aircraft and Cargo Aircraft Limitations (Key 7): Refer to Column 7 of Table A4.1; if P5 is listed, delete "Cargo Aircraft Only". If P1-4 is listed in Column 7, then delete "Passenger and Cargo Aircraft". If the shipment is certified to a Special Approval document which identifies the mode of transportation as Cargo Aircraft Only, delete "Passenger and Cargo Aircraft", even if the PSN is identified as a "P5" in Column 7. If the Special Approval document identifies the mode of transportation as acceptable by either Passenger Aircraft or Cargo Aircraft Only, use the "P" code from Column 7 to determine passenger limitations.
