Shipper's Declaration Form - Second Sequence

The following are the fields to be completed on the form:

Second Sequence - Quantity and Type of Packing (Key 16)

For non-radioactive shipments enter:

  • Number of packages of the same type and content and the type of packaging. The type of packaging listed in this key is the authorized packaging as identified in the packaging paragraph. Identify the type of packaging using the text description of the outer packaging; the UN Specification code is optional.
  • For specifically named self-propelled vehicle and mechanical apparatus, enter the nomenclature or basic description of the item (such as truck, generator, and so on).
  • Weight, or volume, or other applicable measure of the actual hazardous material (per package) for each item with a different proper shipping name, UN ID number, and packing group. Use metric measurements (liter, kilogram, and so on); the English unit can be entered in parentheses following the metric unit. You can abbreviate the quantity's unit of measurement (L for liter, kg for kilograms).
  • Do not include any non-hazardous content of the shipment.
  • If an overpack is used that prevents identification of contents and or UN specification markings, insert the text "Overpack Used" on the Shipper's Declaration form after all relevant entries for the packages contained in the overpack. Identify the number of overpacks if more than one is used. See A16.1.6 for more information.
  • For magnetized material, enter the number and type of packaging; you do not need to enter a net quantity, and the weight and size of the container is optional.
  • The quantity per package immediately after the number and type of package: 2 wooden boxes x 4.5 kg (10 pounds), 1 4G x 5L (1.3 gallons).
  • For magnetized material, enter the number and type of packaging; you do not need to enter net quantity; weight and container size is optional.
  • See Key 16 for information on explosives, and compressed gas (A16.1.4.4-7)
  • For limited quantities Class 2-9, see 16.1.5.
  • If an overpack is used that prevents identification of contents and or UN specification markings, insert the text "Overpack Used" on the Shipper's Declaration form after all relevant entries for the packages contained in the overpack. Identify the number of overpacks if more than one is used. See A16.1.6 for more information.

For radioactive shipments enter:

  • Name or symbol of the radionuclide in the material
  • Description of the physical and chemical form of the material, if not in special form (a generic chemical description is acceptable for chemical form); if a special form is used, enter "Special Form".
  • The number of packages (of same type and content), the type of package, and the activity contained in each package in terms of Becquerel or Terabecquerel (you can include the units of measure such Ci, mCi, or uCi in parentheses).
