Completing Shipper's Declaration - Certification Statement

The next section of the declaration form to complete is the certification statement. This ensures the consignment is acceptable for transport and has been properly prepared in accordance with the DGR. The text for the certification statement is:

"I hereby declare that the contents of this consignment are fully and accurately described above by the proper shipping name, and are classified, packaged, marked and labeled or placarded, and are in all respects in proper condition for transport according to applicable international and national governmental regulations."

In addition, the following statement is required for air transport:

"I declare that all of the applicable air transport requirements have been met."

The signature must include the name and title of the person signing the declaration. This information may be printed or stamped. The place and date of signature is entered next, preferably in the following format: YYYY-MM-DD.
