Completing Shipper's Declaration - Nature & Quantity
The next step is to fill out the Nature and Quantity of Dangerous Goods section. This information is divided into four sequences of data. Each sequence of data must be clearly separated or identified. On computerized forms, this can be accomplished using two oblique strokes or by putting each sequence on a separate line. For manual forms, the sequences can be separated by entering information in the columns provided. The information within a sequence is separated by commas.
The following information must be included in the listed order for the first sequence:
- UN number or ID number
- Proper shipping name
- Class and, if determined, division (include the Compatibility Group letter for Class 1)
- Any assigned subsidiary hazard class or division numbers
- The applicable packing group
For example, the first sequence may appear as follows:
UN 2683, Ammonium sulfide solution, Class 8 (Class 3, Division 6.1), PG II
The second sequence of information must include the following:
- Number of packages, their type of packaging, and the net quantity, as well as the applicable information listed in 8.1.6.9.2(b) through (i) (Note that the number of packages may be indicated using numerical characters or alpha characters.)
- The word "OVERPACK" immediately following the relevant package entries if an overpack is used
The third sequence of information must include:
- The packing instruction number or limited quantity packing instruction number
The fourth sequence of information must include the following information, as applicable:
- Special provision number, if one of the following: A1, A2, A4, A5, A51, A81, A88, A99, A130, A190, or A191
- Statement of approval or exemption associated, if being shipped under government authorization
- Copy of the competent authority for the shipment of portable tanks
- Copy of the competent authority for the shipment, if authorized under 5.0.6.7 for packaging alternatives approved by the State of origin
- Distinguishing sign for motor vehicles in international traffic of the State for explosives listed under Packing Instruction 101
- Statement reflecting approval of organic peroxides and self-reactive substances
