IATA Completing the Shipper's Declaration

The shipper must complete the Shipper's Declaration of Dangerous Goods form. The only sections that may be inserted or amended by a shipper's agent or the operator are the Air Waybill Number, Airport of Departure, or Airport of Destination boxes.

First the shipper's full name and address must be entered, followed by the full name and address of the consignee. These names and addresses may differ from those on the Air Waybill.

Next, enter the Air Waybill number to which the declaration will be attached. In the case of a consolidated shipment, enter the number of the House Air Waybill after the Air Waybill, separated by a forward slash (/).

Then, enter the page number and total number of pages. If there is no extension list, enter "Page 1 of 1 pages."

Next, the shipper must indicate whether the consignment is for "Passenger and Cargo Aircraft" or "Cargo Aircraft Only."

Next, the shipper must indicate the shipment type as radioactive or non-radioactive. Radioactive material must not be included on the same declaration form as other dangerous goods, except for dry ice when used as a refrigerant.

The next step is to fill out the Nature and Quantity of Dangerous Goods section. This information is divided into four sequences of data. Each sequence of data must be clearly separated or identified. On computerized forms, this can be accomplished using two oblique strokes or by putting each sequence on a separate line. For manual forms, the sequences can be separated by entering information in the columns provided. The information within a sequence is separated by commas.

The following information must be included in the listed order for the first sequence:

  • UN number or ID number
  • Proper Shipping Name
  • Class and, if determined, division (include the Compatibility Group letter for Class 1)
  • Any assigned subsidiary hazard class or division numbers
  • The applicable packing group

For example, the first sequence may appear as follows: UN 2683, Ammonium sulfide solution, Class 8 (Class 3, Division 6.1), PG II

The second sequence of information must include the following:

  • Number of packages, their type of packaging, and the net quantity, as well as the applicable information listed in 8.1.6.9.2(a) through (i) of the IATA DGR. (Note that the number of packages may be indicated using numerical characters or alpha characters.)
  • The word "OVERPACK" immediately following the relevant package entries if an overpack is used

The third sequence of information must include:

  • The Packing Instruction number or Limited Quantity Packing Instruction number

The fourth sequence of information must include the following information, as applicable:

  • Special provision number, if one of the following: A1, A2, A4, A5, A51, A81, A88, A99, A130, A190, A191, A201, A202, A211, A212, or A331
  • Statement of approval or exemption is attached if being shipped under government authorization
  • Copy of the competent authority for the shipment of portable tanks
  • Copy of the competent authority for the shipment if authorized under 5.0.6.7 for packaging alternatives approved by the State of origin
  • Distinguishing sign for motor vehicles in international traffic of the State for explosives listed under Packing Instruction 101
  • Statement to reflect approval of organic peroxides and self-reactive substances

The next section of the declaration form to complete is the certification statement. This ensures the consignment is acceptable for transport and has been properly prepared in accordance with these Regulations. The text for the certification statement is:

"I hereby declare that the contents of this consignment are fully and accurately described above by the proper shipping name, and are classified, packaged, marked and labeled/placarded, and are in all respects in proper condition for transport according to applicable international and national governmental regulations."

In addition, the following statement is required for air transport:

"I declare that all of the applicable air transport requirements have been met."

The signature must include the name of the person signing the declaration. This information may be printed or stamped. The date of signature is entered next, preferably in the following format: YYYY-MM-DD.
