Medical Record Contents

29 CFR 2910(f) provides details on the required information that must be recorded in medical records. This information should include (at minimum) the following:

  • The employee's name and social security number
  • The written opinions of the physician or physicians
  • The physician's recommended limitations
  • Exam and test results
  • All employee medical complaints related to exposure to hazardous substances
  • A copy of all information given by the employer to the examining physician (except the CFR standard that the employer is required to provide).
