Injury and Illness Recordkeeping
If employers are required to keep records under OSHA, they must use the OSHA 300, 300A, and 301 Forms.
- OSHA Form 300 (titled "Log of Work-Related Injuries and Illnesses") is an ongoing log of incidents in the workplace and includes details about the employee, the injury or illness, and the time spent away from work. Employers must provide employees, former employees, and their representatives access to this form.
- OSHA Form 300A (titled "Summary of Work-Related Injuries and Illnesses") is a summary of work-related injuries and illnesses. All establishments required to keep records must complete this form, even if no work-related injuries or illnesses occurred. Employers must post this summary from February 1 to April 30 of the year following the year covered by the form (OSHA Form 300A).
- OSHA Form 301 (titled "Injury and Illness Incident Report") is the detailed form that employers must complete each time a workplace injury or illness occurs. It includes confidential information about the medical treatment provided. Employers must complete this form for every injury.
Employers must provide access to employee medical records and exposure records to employees or their authorized representatives.
