Employer Responsibilities - Incident Forms

For those employers who are required to keep records under OSHA, the injury and illness record-keeping forms are as follows:

  • "Log of Work-Related Injuries and Illnesses" (OSHA Form 300). This is an ongoing log of incidents in the workplace and includes details about the employee, the injury or illness, and the time spent away from work. Employers must provide employees, former employees, and former employees' representatives access to this log.
  • "Summary of Work-Related Injuries and Illnesses" (OSHA Form 300A). This is a summary of work-related injuries and illnesses. All establishments required to keep records must complete this form, even if no work-related injuries or illnesses occurred. Employers must post this summary from February 1 to April 30 of the year following the year covered by the form.
  • "Injury and Illness Incident Report" (OSHA Form 301). This is the detailed form that employers must complete each time a workplace injury or illness occurs. It includes confidential information about the medical treatment provided. Employers must complete this form for every injury.

Employers must provide access to employee medical records and exposure records to employees or their authorized representatives.
