Recordkeeping & Reporting Requirements
Employers with 11 employees or more must keep records of serious work-related injuries and illnesses and make these available to employees. OSHA excludes employers with 10 or fewer employees and employers in certain low-hazard industries from this requirement.
Minor injuries requiring first aid only do not need to be recorded.
Employers must keep accurate records of medical screening and surveillance. This includes recording the following information:
- The name and Social Security number of the employee
- The physician's written opinions, recommended limitations of exposure for hazardous substances, and results of examinations and tests
- Any employee medical complaints related to exposure to hazardous substances
- A copy of the information provided to the examining physician by the employer
