Equipment Modifications

Manufacturers may recommend upgrades, including software upgrades, that require changes to be made to safe operating procedures. An updated hazard evaluation is required if modification to an HCLS affect the claims made by its label. Because modifications may introduce the need for changes in operating procedures that are difficult to anticipate, all modifications should be made with extreme caution. It is the responsibility of the LSO to ensure that a hazard evaluation is completed after modifications and any necessary changes are made to the policies and procedures.
