Additional Requirements for LOTO Procedures

If energy-isolating devices are used in remote or non-contiguous locations, such as offshore oil and gas production facilities, documented procedures for lockout/tagout must be followed in conjunction with prior written authorization. This helps to ensure that authorized individuals performing the work verify the isolation and de-energization or re-energization through direct communication with individuals authorized to perform the lockout procedures.

All energy-isolating devices must be locked and the keys for the devices kept by a lockable device that is locked by the sole person designated by the procedures to establish the isolation on behalf of the authorized individual.

Shift Changes

Established lockout/tagout procedures must be used during shift or personnel changes to ensure the continuity of lockout (or other forms of hazardous energy protection), including a documented procedure for transferring lockout or device protection from departing to incoming authorized individuals to minimize hazard exposure from the unexpected energization or start-up of the machine, equipment, or process or from the release of stored energy.
