Group Lockout

When more than one individual is involved in lockout/tagout procedures, group lockout requirements must be observed. Each energy-isolating device must be secured by a single lockout in accordance with the following steps:

• A primary authorized individual (typically a supervisor or related position) is assigned responsibility for locking out each energy-isolating device.
• The keys to the lockout devices are controlled by a lockable device, such as a lock box or key ring; this device must be locked by the primary authorized individual.
• Once the lockout is applied, energy isolation must be verified per company procedures.
• Before any authorized individuals being work, they must first:
  • Familiarize themselves with the energy-isolating devices to be used.
  • Assess their level of adequacy for the work effort using documented lockout procedure.
• Authorized individuals may then apply their personal lock (and tag if applicable) to the lockable device; each authorized individual should request that their isolation is verified by another individual in their presence.
• Work may commence only after all involved authorized individuals have applied their personal lock (and tag if applicable) to the lockable device and are certain that all hazardous energy has been isolated and locked out.
• All non-essential items should be removed from the work site as individuals complete their duties; when individuals are certain their job duties are complete, they may remove their personal lock (and tag if applicable) from the lockable device.
• Once all authorized individuals have removed their personal locks and tags from the lockable device, the primary authorized individual responsible for the lockout device must inspect the work area to ensure that all employees are clear before removing the locks from the isolating device and re-energization occurs.
