Program Documentation
CSA standards dictate that employers must create documentation recording each task and its associated hazards; this documentation must be kept and made available to all employees. The objective of this standard is to encourage employers to create a documentation tool that is useful to both managers and workers.
Where applicable, the documentation should include risk assessments for each task. These assessments must include the following information:
- The machine, equipment, or process for which the assessment was made and any relevant assumptions made
- The hazards identified as follows:
- Hazardous situations identified
- Hazardous events considered
- The information on which the assessment was based as follows:
- The data used and the sources
- Any uncertainty associated with the data and its impact on the risk assessment
- The objectives behind any and all protective measures
- The protective measures taken to eliminate identified hazards or reduce risk
- The residual risk associated with the machines, equipment, or processes
- The result of the final risk evaluation and any relevant forms completed throughout the assessment
