Lockout/Tagout

Lockout/tagout (LOTO) is perhaps the most important part of controlling hazardous sources of energy. It should be the first line of defense when working around electricity. Even if the task at hand is a "quick" job, miners must always follow LOTO procedures.

By definition, LOTO comprises the practices and procedures necessary to disable machinery before performing maintenance tasks. In lockout, the equipment is locked to render it inoperable while workers perform maintenance tasks. In tagout, a tag is affixed to the locked device to warn other workers and prevent them from starting the equipment. Maintenance employees can be exposed to serious hazards if equipment is not properly locked or tagged.

Establishing a LOTO procedure and its accompanying documentation is the sole responsibility of the employer. These steps should be specific to the worksite and clearly communicated to all present and future employees.

The following MSHA provisions, found in 30 CFR 56.12016 and 56.12017, are standard LOTO practices for all surface mines:

  • Disable electrically powered equipment before performing any mechanical work.
  • Disable power circuits, unless hot-line tools are used for maintenance.
  • Lock out power switches or use other methods to ensure that the equipment is not turned on while maintenance is being performed.
  • Post warning notices at the power switch (the notice must be signed by the maintenance worker performing the task).
  • Ensure that only the maintenance worker who installed the lock and tag or an authorized person removes the lock and tag after maintenance is complete.
