Procedures for Nominating a Representative
Miners may nominate a representative either by mail or online through MSHA's website.
By Mail
The nomination form, officially known as MSHA Form 2000-238, may be downloaded from MSHA's official website. Only three items are required to submit the form:
- The information of at least two nominating miners (if requested, this information can be kept confidential)
- The nominated representative's information
- The mine ID and the mine operator's information
The signature of the nominated representative is required to complete the form. Once completed, the form should be mailed to your local MSHA district office.
Online
The same three items of information required for the mailed form are required for the online form.
For all online forms, note that a valid email address for the nominated representative is required in order for the representative to "sign" the form.
Once a representative has been approved, the mine operator must post a copy of the representative's information on the mine bulletin board.
