Training Records

Training records must be created for certification purposes. According to §48.9 and §48.29, operators are required to give a copy of the training certificate to miners upon completion of training. Certification may be recorded on a certificate of training form called MSHA Form 5000-23 or an approved alternative. Copies of training certificates for currently employed miners must be kept at a mine site for two years, or for 60 days after employment has been terminated.

Records must include the following:

  • Full name of the person trained
  • Type of training
  • Duration of training
  • Date the training was received
  • Name of the person who provided the training
  • Name of mine or independent contractor
  • MSHA mine identification number or independent contractor number
  • Location of training
