PRCS Entry Permit

If an employer determines that a confined space is fit for entry, a confined space permit must be implemented. This permit is a mandatory checklist that you must follow throughout the entry process. Permits have a definite duration and are cancelled when the entry is over or if an emergency develops and there is a necessary evacuation of the space. Permits are not required for rescue operations within a confined space. The following components are included within a permit:

  • Identity, location, and purpose of a permit space
  • Purpose of the entry
  • Date of entry and authorized duration
  • List of authorized entrants, attendants, and name and signature of the entry supervisor
  • Hazards present in the permit space
  • Hazard control and isolation techniques
  • Results and timing of tests performed, as well as names of testers
  • Communication, rescue, and emergency response procedures
  • Equipment provided for entry into the space
  • Acceptable entry conditions
  • Other permits, such as a hot work permit

Upon completion, your supervisor will provide you with a copy of the permit, or it will be posted at the entrance of the confined space, so that you may understand completely the role that you have throughout the entry process. Entry permits must be retained for one year.
