Respirator Medical Evaluation Questionnaire Components
OSHA's Medical Evaluation Questionnaire contains several sections.
Part A, Section 1: This mandatory section includes general questions (such as name, age, weight, and height). It also includes general information about the employee's background with respirator use.
Part A, Section 2: This mandatory section includes nine yes/no questions for employees who have worn respirators before. It also includes an additional six questions for employees who will wear a full-facepiece respirator or a SCBA.
Part B: This section includes specific questions, such as information related to prior exposure to hazardous materials, previous occupations, and potentially pertinent details for determining if the employee can safely wear a respirator (for example, if the employee has ever been exposed to asbestos, and if he or she has ever served in the military). The licensed medical professional will decide if this needs to be completed.
