Respirator Medical Evaluation Questionnaire Requirements

The following are requirements related to the medical evaluation and questionnaire:

  • The employer must name a physician or other licensed healthcare professional (called a PLHCP) to conduct the medical evaluation.
  • The PLHCP must use the medical questionnaire in Appendix C of the Respiratory Protection standard, or a medical examination with the same information may substitute for the questionnaire. This substitute must obtain all of the information asked in Sections 1 and 2, Part A of Appendix C. It may also include questions in Part B of Appendix C if the PLHCP deems this necessary.
  • The questionnaire and examination(s) must be completed confidentially and during the employee's normal working hours.
  • The questionnaire and examination(s) must be completed at a time and place convenient to the employee.
  • Employees must understand the purpose and content of these procedures.
  • The questionnaire must be provided directly to the PLHCP.
  • The employer cannot review the employee's responses.
  • The employer must give the employee a chance to discuss the questionnaire and examination results with his or her PLHCP.
